目的:评价外科治疗马凡综合征合并急性Standford A型夹层的效果。方法:回顾性分析2007年7月至2010年7月外科治疗12例马凡综合征合并急性Standford A型夹层病例的临床资料。采用改良Mini-root手术方式代替Bentall手术。结果:主动脉阻断时间69~103 min,平均(78.7±28.6)min,体外循环时间79~122 min,平均(98.3±23.8)min。术后早期存活11例,死亡1例,死亡率8.3%。术后随访时间2~37个月,平均(18.2±8.6)个月,其中1例术后24月发生急性腹主动脉夹层,予以实施腔内支架隔绝术成功以治愈出院。术后与术前左心室舒张末径分别为45~58 mm,平均(50.2±5.6)mm和53~69 mm,平均(61.3±4.6)mm(P<0.01)。结论:马凡综合征合并急性Standford A型夹层一经确诊则需急诊手术,及时的外科手术是治疗该病的有效方法,而且应用改良Mini-root手术方式疗效满意。  相似文献

Various research studies have been conducted in deducing the role of microRNAs (miRNAs) in the pathogenesis and physiological processes of various systematic diseases. This study aims at demonstration of the important role played by miR-29a-3p, through association with phosphatase and tensin homolog (PTEN), in the regulation of abdominal aortic aneurysm development and progression. Quantitative real-time polymerase chain reaction (RT-qPCR) examined miRNA-19a-3p and PMEPA1 expression in multiplied vascular smooth muscle cells (VSMCs). Cell transfection upregulated or downregulated the genes and cell counting kit-8 assay determined cellular viability. RT-qPCR detected cellular proliferation and cell death using the cell proliferation and apoptosis biomarkers Ki87 and proliferating cell nuclear antigen, caspase-8 and caspase-3, respectively. Furthermore, luciferase assay analyzed the luciferase activity and western blot analysis determined miRNA-19a-3p and PMEPA1 protein expression in proliferation and apoptosis biomarkers. TargetScan 4.2 online software ( www.targetscan.org ) was used to perform the bioinformatics analysis so as to forecast the putative targets of miR-29a-3p and PTEN. The results inferred that there was an increased expression of miRNA-29a-3p found in AAA-mimic cells with increased cellular viability and significant pathological apoptosis. Further, when the expression of miRNA-29a-3p was downregulated, it reduced the cell viability of AAA cells. On the basis of the gene interplays, it can be understood that the PTEN was directly targeted by miRNA-29a-3p so as to regulate the AAA progression. Thus, PTEN was found to strengthen the proliferation effect of miRNA-29a-3p in AAA cells. The current study thus shed more insights about the molecular mechanistic roles of miRNA-29a-3p and PTEN, opening doors for novel therapeutic approach to AAA.  相似文献

该文主要探究了LPS通过上调骨形态发生蛋白4(bone morphogenetic protein 4,BMP4)促进猪主动脉瓣膜间质细胞(valve interstitial cells,VICs)成骨样分化的作用及机制,为钙化性主动脉瓣膜病(calcific aortic valve disease,CAVD)的干预及治疗提供理论依据。采用免疫组化法检测非CAVD组和CAVD组瓣膜组织中Runx2和BMP4的表达,Western blot检测Runx2和BMP4的蛋白表达;采用胶原酶(I型)消化瓣膜后分离猪VICs,用免疫荧光染色进行表型鉴定;采用LPS和重组人BMP4腺病毒处理VICs;用ALP染色、茜素红S染色、qRT-PCR和Western blot法检测细胞的早期和晚期成骨能力、Smad1/5/8和ERK1/2的磷酸化水平。结果显示,BMP4和Runx2蛋白在CAVD组中的表达水平明显高于non-CAVD组。原代猪VICs分离成功,其中α-SMA和Vimentin呈阳性,CD31呈阴性。LPS可使VICs ALP活性增强、钙盐沉积增多、钙化指标上升和BMP4增加;BMP4可使VICs ALP活性增强、钙盐沉积增多、钙化指标上升,且使Smad1/5/8和EKR1/2的磷酸化水平升高。提示LPS可以上调BMP4的表达进而促进VICs的成骨样分化,Smad1/5/8与ERK1/2信号通路可能在该过程中发挥重要作用。  相似文献

Endothelial cells (ECs) play many roles in vascular biology, including control of blood pressure, blood clotting, atherosclerosis, angiogenesis, and inflammation. Gap junctions (GJs) are channel-like assemblies of connexin (Cx) family proteins that connect neighboring cells and modulate and synchronize their intracellular environments by the transfer of intracellular mediators. It has been reported that vascular ECs express Cx37, Cx40, and Cx43, but not Cx32. Here, we showed that Cx32 mRNA and protein are expressed in various cultured human ECs. We confirmed Cx32 expression in blood vessel ECs using wild-type and Cx32 knock-out mice. We observed that dye transfer between cultured ECs through gap junctions is suppressed by an anti-Cx32 monoclonal antibody. These findings suggest that vascular ECs express Cx32, which participates in endothelial gap-junction intercellular communication.  相似文献

Treatment options for abdominal aortic aneurysm (AAA) include highly invasive open surgical repair or minimally invasive endovascular aneurysm repair (EVAR). Despite being minimally invasive, some patients are not suitable for EVAR due to hostile AAA morphology. Fenestrated-EVAR (F-EVAR) was introduced to address these limitations of standard EVAR, where AAA is treated using a Fenestrated Stent Graft (FSG). In order to assess durability of F-EVAR, displacement forces acting on FSGs were analysed in this study, based on patient-specific geometries reconstructed from computed tomography (CT) scans. The magnitude and direction of the resultant displacement forces acting on the FSG were numerically computed using computational fluid dynamics (CFD) with a rigid wall assumption. Although displacement force arises from blood pressure and friction due to blood flow, numerical simulations elucidated that net blood pressure is the dominant contributor to the overall displacement force; as a result, time dependence of the resultant displacement force followed pressure waveform very closely. The magnitude of peak displacement force varied from 1.9 N to 14.3 N with a median of 7.0 N. A strong positive correlation was found between inlet cross-sectional area (CSA), anterior/posterior (A/P) angle and the peak displacement force i.e. as inlet CSA or A/P angle increases, the magnitude of resultant displacement increases. This study manifests that while loads exerted by the pulsatile flow dictates the cyclic variation of the displacement force, its magnitude depends not only on blood pressure but also the FSG morphology, with the latter determining the direction of the displacement force.  相似文献

Despite increased social awareness, marketing restraints, tobacco taxation, and available smoking cessation rehab programs, active and passive smoking remain a worldwide challenging epidemic and a key risk factor for cardiovascular diseases development. Although cardiovascular (CV) protection is more pronounced in women than in men due to estrogenic effects, tobacco cigarette smoking exposure seems to alter this protection by modulating estrogen actions via undefined mechanisms. Premenopausal cigarette smoking women are at higher risk of adverse CV effects than non-smokers. In this study, we investigated the impact of cigarette smoking on early CV injury after myocardial infarction (MI) in non-menopausal female mice. Aortic arch calcification, fibrosis, reactive oxygen species, and gene expression of inflammatory and calcification genes were exaggerated in mice exposed to cigarette smoke (CS). These findings suggest that aortic injury following MI, characterized by vascular smooth muscle cells transdifferentiation, calcification, inflammation, and collagen deposition but not cardiac dysfunction is exacerbated with CS exposure. The novel findings of this study highlight the importance of aortic injury on short and long-term prognosis in CS-exposed MI females. Linking those findings to estrogen alteration is probable and entails investigation.  相似文献

Clinical and experimental studies show that angiotensin II (AngII) promotes vascular pathology via activation of AngII type 1 receptors (AT1Rs). We recently reported that NP‐6A4, a selective peptide agonist for AngII type 2 receptor (AT2R), exerts protective effects on human vascular cells subjected to serum starvation or doxorubicin exposure. In this study, we investigated whether NP‐6A4–induced AT2R activation could mitigate AngII‐induced abdominal aortic aneurism (AAA) using AngII‐treated Apoe?/? mice. Male Apoe?/? mice were infused with AngII (1 µg/kg/min) by implanting osmotic pumps subcutaneously for 28 days. A subset of mice was pre‐treated subcutaneously with NP‐6A4 (2.5 mg/kg/day) or vehicle for 14 days prior to AngII, and treatments were continued for 28 days. NP‐6A4 significantly reduced aortic stiffness of the abdominal aorta induced by AngII as determined by ultrasound functional analyses and histochemical analyses. NP‐6A4 also increased nitric oxide bioavailability in aortic tissues and suppressed AngII‐induced increases in monocyte chemotactic protein‐1, osteopontin and proteolytic activity of the aorta. However, NP‐6A4 did not affect maximal intraluminal aortic diameter or AAA incidences significantly. These data suggest that the effects of AT2R agonist on vascular pathologies are selective, affecting the aortic stiffness and proteolytic activity without affecting the size of AAA.  相似文献

In an aortic smooth muscle cell line, A10 cells, we investigated the effect of sphingosine 1-phosphate on the induction of heat shock protein 27 (HSP27), a low-molecular-weight heat shock protein. Sphingosine 1-phosphate significantly induced the accumulation of HSP27 in a pertussis toxin-sensitive manner. The effect was dose-dependent in the range between 0.1 and 30 microM. Sphingosine 1-phosphate stimulated an increase in the levels of mRNA for HSP27. Sphingosine 1-phosphate stimulated both p42/p44 mitogen-activated protein (MAP) kinase and p38 MAP kinase activation. PD98059, an inhibitor of the upstream kinase that activates p42/p44 MAP kinase, did not affect sphingosine 1-phosphate-stimulated HSP27 induction. In contrast, SB203580, an inhibitor of p38 MAP kinase, reduced sphingosine 1-phosphate-induced HSP27 induction. SB203580 reduced the levels of mRNA for HSP27 induced by sphingosine 1-phosphate. These results indicate that sphingosine 1-phosphate stimulates the induction of HSP27 via p38 MAP kinase activation in aortic smooth muscle cells.  相似文献
